What Is Type 2 Diabetes?

Type 2 diabetes is a long-term condition in which the body does not use insulin properly, so sugar (glucose) builds up in the blood instead of moving into the cells for energy. It is the most common form of diabetes, and it usually develops gradually in adulthood.

Doctors track type 2 diabetes with a blood test called HbA1c, which reflects average blood sugar over roughly the past three months. Over time, blood sugar that stays too high can affect the heart, blood vessels, kidneys, eyes and nerves. Type 2 diabetes is usually managed by a primary care physician or an endocrinologist with diet, exercise, oral medications and, for some people, insulin.

This clinical research study is enrolling adults with type 2 diabetes in three groups: people managing with diet and exercise alone, people on a stable dose of metformin, a sulfonylurea or an SGLT2 inhibitor, and people on a stable dose of basal (long-acting) insulin.

Understanding Type 2 Diabetes and Blood Sugar

With type 2 diabetes, the body either does not make enough insulin or does not respond to it well, a problem called insulin resistance. Without enough working insulin, glucose stays in the bloodstream and blood sugar levels rise.

Two common measures help doctors understand how type 2 diabetes is being managed:

  • HbA1c — a blood test that reflects average blood sugar over about three months
  • Blood glucose — a fingerstick or lab reading of blood sugar at a single point in time

Some people with type 2 diabetes notice no symptoms at first. When symptoms do appear, they may include:

  • Increased thirst and frequent urination
  • Feeling tired or low on energy
  • Blurred vision
  • Cuts or sores that are slow to heal
  • Tingling or numbness in the hands or feet

Type 2 diabetes is typically managed with healthy eating, physical activity and weight management, and many people also take oral medications, non-insulin injections or insulin. Clinical research studies help evaluate investigational approaches that may add to the options available in the future.

Important: never stop or change a prescribed diabetes medication or insulin dose on your own. Talk with your doctor or the study team first.

Frequently Asked Questions

Type 2 diabetes clinical trials at CRWF are research studies that evaluate investigational treatments for adults living with type 2 diabetes. In this study, participants are assigned at random to receive either the investigational once-weekly injection or a placebo, so researchers can measure the true effect of the study medication.

You may qualify if you are 18 or older, have been diagnosed with type 2 diabetes, and have a BMI of at least 23 kg/m². The study is enrolling adults who manage with diet and exercise alone, adults on a stable dose of metformin, a sulfonylurea or an SGLT2 inhibitor, and adults on a stable dose of basal insulin. Full eligibility, including HbA1c levels, is confirmed during a screening visit at our Tampa or Clearwater research facility.

No. Do not stop or change any medication or insulin dose on your own. This study includes groups for people on no diabetes medication, people on stable oral medication and people on stable basal insulin, and the study doctor will review everything you take during screening. Keep your regular doctor informed as well.

There is no cost to participate. If you qualify, study treatment and study-specific visits and tests are provided at no cost. Participation does not replace your relationship with your regular physician.

Yes. Qualified participants may receive up to $3,000 for completed study visits for their time and travel: $250 for each of up to 12 site visits, $150 for each phone visit completed instead of a site visit, plus $100 for each eye exam. The details, including how and when it is paid, are explained during the informed consent process before you agree to enroll.

This type 2 diabetes study lasts about 1 year and 2 months and includes up to 12 site visits and 9 phone calls with our Tampa or Clearwater study team. Visits typically involve health checks, lab work and a review of any symptoms or medications. Participation is voluntary and you may withdraw at any time.

Before anything else, you will review and sign an informed consent form that explains the study in plain language, including the potential risks and benefits. After that, the study team performs screening assessments, which may include blood tests such as HbA1c, a physical exam and a review of your medical history and medications, to confirm that the study is an appropriate fit for you.

Your Safety During The Study

Safety is the top priority throughout a type 2 diabetes clinical trial. Your blood sugar and overall health are monitored throughout the study, the research team is available between visits to answer questions, and any side effects are documented and reviewed. You are free to withdraw from the study at any time, for any reason, without affecting your regular medical care.
